Fatty Liver Progression

NAFLD progresses to NASH (20-30%), then Fibrosis (40-50%), Cirrhosis (10-15%), and HCC (1-3%/year). Early detection can halt or reverse liver damage.

NAFLD (Non-Alcoholic Fatty Liver Disease)

In India NAFLD Estimated 25-32% of general population and Global prevalence is 25-30%.

  1. Simple Steatosis - 70-80% of NAFLD patients fat accumulation but low risk of progression
  2. NASH (Non-Alcoholic Steatohepatitis) - 20-30% of NAFLD patients inflammation + liver injury and higher risk of progression

NAFLD to NASH

20-30% of NAFLD patients progress to NASH over time. Risk factors accelerating progression:

  1. Type 2 Diabetes Mellitus
  2. Obesity and Metabolic Syndrome
  3. Older age, Male sex, and Genetic predisposition

NASH to Fibrosis

Among NASH patients, 40-50% may develop some stage of fibrosis. Fibrosis Stages:

  1. F0 = No fibrosis
  2. F1 = Mild (periportal)
  3. F2 = Moderate (portal fibrosis)
  4. F3 = Severe (bridging fibrosis)
  5. F4 = Cirrhosis
  6. 15-25% may develop fibrosis (F2-F3) over 10-15 years
  7. 10-15% may progress to cirrhosis (F4) over 15-20 years

Cirrhosis and Hepatocellular Carcinoma (HCC)

5-20% of NASH patients may progress to cirrhosis over time. Once cirrhosis develops, Decompensation risk: 5-7% per year.

  1. Annual incidence ~1-3% in NASH cirrhosis
  2. HCC can also develop without cirrhosis in NASH (20% of NASH-HCC cases occur in non-cirrhotic livers)
  3. NASH is now the 2nd leading cause of liver transplantation for HCC in some countries

Progression Pathway

  1. NAFLD to NASH - Progression: 20-30%
    Time Frame: 1-10Years
  2. NASH to Fibrosis - Progression: 40-50%
    Time Frame: 5-15Years
  3. Fibrosis to Cirrhosis - Progression: 10-15%
    Time Frame: 10-20Years
  4. Cirrhosis to HCC - Progression: 1-3% annual risk

What is pre-diabetes?

Pre-diabetes is a metabolic state where blood sugar levels are higher than normal but not high enough to be diagnosed as diabetes. It signals a high risk of developing Type 2 diabetes.

What causes fatty liver disease?

Fatty liver disease occurs when excess fat builds up in liver cells, often due to obesity, insulin resistance, high cholesterol, or excessive alcohol intake.

How can pre-diabetes be reversed?

What are common symptoms of diabetes?
